The Rutland Snowdrop Award -
Celebrating Compassionate Care

Application Form

The Rutland Snowdrop Award recognises and celebrates exceptional people who work in a caring profession.

Do you know a professional health or care worker or a care team who made a real difference to someone in their final days and weeks, by going 'above and beyond' to ensure this person had the best end of life possible?

You can nominate a member of staff in a care home, a medical professional, a nurse, or social worker or a group of care professionals for the Rutland Snowdrop Award.

Whilst the Care Quality Commission evaluates the quality and standards of care, the Rutland Snowdrop Award focuses on the human side of care and recognises people who work in care who go 'above and beyond' their day to day work responsibilities to help improve the end of life experience of individuals and their families. The Award celebrates people who work in care who show true compassion and understanding and who are able to make a positive difference to those they care for.

* If you would like to nominate an individual or a team of care workers, please ensure that that you have received permission before submitting any of their personal details (such as their name and contact details) to us.

Please state below the reason(s) why you would like to nominate an individual health or care worker or a team of professionals for the Rutland Snowdrop Award.

Please provide examples of how your nominee made a difference by going  'above and beyond' their day to day job responsibilities to assist someone during their end of life. Please also include the person being cared for (e.g. a family member, a friend or a neighbour).

Please note that we may wish to call or email you to go through some of the details you have provided.

The details of all nominees will be put forward to a panel of judges who will announce the winners of the Snowdrop Award at an Annual Award Ceremony.
